We are excited to announce that we have a newly created Care Manager (Grade 5 RN) role at our rural property, Homestead Estate Home located in Wallington, Geelong

 

Situated in rural Wallington on the Bellarine Peninsula, Homestead Estate is set in an idyllic rural location with views to the surrounding countryside.  

With 190 beds, Homestead Estate has recently completed an extensive renovation to introduce a unique range of services and facilities.

 

You will thrive being on the floor with a high performing team in this hands-on role to provide our residents with exceptional care while maintaining positive relationships that contribute to a safe and caring environment.


What you’ll be doing

  • Contribute to continuous clinical improvement to delivery excellence in care
  • Support residents, families and visitors in a highly professional manner
  • Ensure assessments, care plans and reviews are undertaken appropriately, on time and well documented
  • Manage clinical education and competency of staff 
  • Promote and foster a positive, collaborative and diverse culture 

Hours of Work

  • Monday to Friday
  • 0800 – 1615 (or variation depending on operational needs)

Things that matter to us 

  • Two years’ experience in a clinical leadership role 
  • Understanding of the Aged Care Quality Standards   
  • Relevant Nursing Qualifications (Division 1) 
  • Current registration with AHPRA 
  • Up-to-date COVID-19 vaccinations (3 doses)    
  • 2024 influenza vaccination    
  • NDIS Check (includes Police Clearance)    
  • Eligibility to work in Australia
